Privacy Policy
GTX Therapy ("GTX," "we," "us") is the telehealth concierge practice of Miguel A. Martinez, MD. This policy describes how we collect, use, and protect information when you visit this website or communicate with us. Protected health information you provide as a patient is additionally governed by our HIPAA Notice of Privacy Practices, which is provided to you when you become a patient and is available on request.
Information we collect
- Contact information you provide — such as name, email address, and phone number — when you request information or begin an assessment.
- Health information you share with the practice through its forms or communications once you become a patient, which we treat as confidential and handle under HIPAA. The symptom assessment on this website is informational only; your responses are not stored or transmitted.
- Referral information. If you enter a referral code when joining, we record the code, your name and email address, and the membership plan you selected so that the referring member receives credit. Referring members can look up only their own signup count and credit totals; they never see your name or contact information.
- Enrollment and payment information. Membership enrollment, including identity, contact, and payment details, is completed on Elation Health, our secure patient platform, which operates under its own privacy practices and a Business Associate Agreement with the practice. This website does not receive or store your payment information.
- Technical information such as browser type and pages visited, used to keep the site working and secure. Preferences such as your language selection are stored in your browser. We do not use advertising cookies, and any web analytics we use is configured not to collect health information.
How we use information
- To respond to inquiries, schedule visits, and provide care.
- To administer memberships, billing, and appointments.
- To operate, secure, and improve this website.
- To send administrative or service communications. We do not sell your personal information, and we do not use patient information for third-party advertising.
How we share information
We share information only with service providers who help us operate — such as cloud hosting (Microsoft Azure), network security and content delivery (Cloudflare), our patient platform (Elation Health), and pharmacy partners — under agreements that require them to protect it, and as required by law. Where a vendor handles protected health information on our behalf, we enter into a Business Associate Agreement with that vendor as required by HIPAA.
Links to other sites
This site links to Elation Health for enrollment and the patient portal, and to Function Health as an optional source for laboratory testing. Those sites have their own privacy policies. If you sign up with Function Health through our link, Function Health may attribute your signup to GTX Therapy; we do not receive your lab results through that link unless you choose to send them to us.
Your choices and rights
You may request access to, correction of, or deletion of your personal information, subject to legal record-keeping requirements that apply to medical practices. California residents have additional rights under the CCPA/CPRA, including the right to know and the right to non-discrimination for exercising privacy rights. To make a request, contact us using the information below.
Security and retention
We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information we hold, including encryption in transit and at rest. Medical records are retained as required by California law.
Children
Our services are for adults 18 and older. We do not knowingly collect information from minors.
Changes
We may update this policy from time to time; the effective date above reflects the latest revision.
